In Sweden many apartments in the cities have fiber in the house. I live in an apartment built in 1969, and pay about 50 USD for 100/10 mbit fiber with a static IP address.(100/100 is available but costs more than twice as much.) It's never been down, even for a minute, and there was no fee to "install" it. (Instead I got a wlan router for free when I signed up for a year.) All I had to do to get it working was to plug in the network cable in the jack in the wall and fill in the form on the default homepage that dhcp gave me.

You can also get cable 24/2 for about 40 USD and DSL 10/1 for around 30 USD.

In Germany providers and cable companies are split. So we have a great competition between the ISP cause they pay just a fixed sum for using the wires.

It makes it possible to get ADSL2+ with 16.000kbit and 1204kbit up for 20

Choice for ADSL in the UK is vast but unfortunately all the cable companies bought each other out until there was only one left. I wanted to save myself the cost of a fixed line by using VoIP so I went with cable. The service is okay, apparently better than their TV service (which I don't use). The only gripe is that I get a bit of packet loss on VoIP calls but my new phone seems to handle it a lot better than the old one.
