I first pitched the includes proposal back in March 2007
You weren't first to think of that. The idea is as old as the handbook, just a bit older actually.
It did take quite a bit of XSL hacking, the most part being in http://tiny.wistful.net/id/3056 and http://tiny.wistful.net/id/3057
I am pleased it is being appreciated and put to good use. Thanks for all the work you put into this release.

Hmm, how come it never really gained a foothold in our docs then? It seems logical to use it everywhere in our docs, but I'd never seen it used until recently. Was there a decision made to deliberately not use it at some point?

Oh, and your XSL-fu is amazing. :)
